Anonomously Report Child Safety Concerns

Sesame Lane takes every concern about the safety and wellbeing of children seriously. You do not need proof before raising a concern, and you may submit this form without providing your name or contact details.

Information submitted may need to be shared with Queensland Police, Child Safety or another relevant authority to protect a child or meet our legal obligations.

Please describe any immediate or ongoing safety concerns.

If anyone is in immediate danger or the situation is life-threatening, call Triple Zero (000) immediately. This form is not monitored continuously and should not be used for emergencies.

If you are unsure, please provide any information you have.

3. Please describe your concern.

Tell us what you saw, heard, were told or believe may be happening. Please include as much detail as you can. If a child told you something concerning, please include their exact words if you can remember them.

4. Who is involved?

Please provide the names, roles or descriptions of the child, employee or other people involved, if known.

5. When and where did this happen?

Please provide the date, time and location if known. An approximate date, time or location is still helpful.

6. Were there any witnesses or other people who may know what happened?

Please provide their names or descriptions if known.

7. Is there any other relevant information or evidence we should be aware of?

This might include messages, photographs, CCTV, documents, previous incidents or other concerns. Please do not upload or send child sexual abuse material or intimate images. This material must be reported directly to Queensland Police.
